One of the most daunting issues for family members of a loved one with a disability is ensuring continuity of care when a primary caregiver or advocate must change. How can you be sure that the proper care and consideration for your loved one’s needs will continue after you’re no longer able to provide it? The Letter of Intent is a guidance document that details your loved one’s unique history, experiences, habits, housing, employment, recreation, spiritual, medical information, and more. It states your wishes and theirs for the future.

With her guidance, you will be able to put into writing everything you need to create a secure future for your loved ones with special needs.
