Please prepare a check with the appropriate amount payable to “Foundation of The Arc of Northern Virginia”. Mail check to: 3060 Williams Drive, Suite 300, Fairfax VA 22031. Attention "Assistant Dir of Trust" on the envelope.

MAIL CHECK

If you are establishing a trust, here’s what you’ll need to share with us prior to your appointment:

  • Copies of Photo IDs for trust beneficiary and grantors.
  • Copies of beneficiary's insurance cards: medical, vision, dental.
  • Copy of a recent letter from Social Security Administration or a physician's statement signed and dated by diagnosing doctor.
  • Contact information for other people who will be involved in various ways with the trust.
